Are you ready for eCQM mandates?

Is your organization prepared for the Hypoglycemia and Hyperglycemia eCQM’s?
New rules mandate hospitals begin tracking:

  • Severe hypoglycemia – defined as the percentage of patients having one blood glucose reading less than 40 mg/dl during their stay, within 24 hours of administration of insulin (or other antihyperglycemic agents).

  • Severe hyperglycemia – defined as the percentage of hospital days with one blood glucose reading greater than 300 mg/dL (excluding the first 24-hour period after admission)

TransformativeMed’s Core Diabetes application can quickly and easily help you improve what really matters: your clinical outcomes and improved patient care.

Core Diabetes Event Series

We are hosting events focused on managing SubQ and IV insulin, the use of data and analytics for QI efforts, predicting hypoglycemia risk using artificial intelligence and machine learning, and other timely topics as your organization prepares for the new eCQM diabetes requirements for acute care.
These events will include TransformativeMed clinical and technical experts, clinical partners like Pronia Health, current & new clients, and other stakeholders responsible for managing and improving diabetic care in the hospital.
Upcoming events

Events List

  • Jun 14, 10:00 AM – 11:00 AM PDT
A review of strategies and EHR-embedded resources for carb-based SubQ insulin dosing in the hospital.  Discuss carb ratios, correction factors, and other approaches for carbohydrate-matched insulin dosing and administration.  Share challenges and consider solutions with other clinicians.
  • Jun 28, 10:00 AM – 10:30 AM PDT
Prescribing insulin for acute care patients can be complex. Fasting for procedures, use of steroids, and other rapidly changing conditions increase the difficulties providers face while trying to gain proficiency with insulin management.
  • Jun 28, 1:00 PM – 1:45 PM PDT
Discuss how EHR data and workflows can be combined with AI/ML tools to predict hypoglycemia and other risk factors. Talk with colleagues about how predictive risk scores can be used in clinical workflows to make managing complex disease processes easier and safer.
