Problem:
Each hospital service used its own version of a rounding and communication tool. There was not a complete, unified picture of the patient’s course of hospitalization, and the administrative overhead was taking time away for direct patient care.
Solution:
Implementation of TransformativeMed's Core Work Manager tool led to a centralized pre-rounding prep, rounding, patient hand off, and communication system which ended up saving approximately 45minutes per provider.
